• First Month $125 plus $175 Registration/Intake Fee
• Subsequent Months $125/month
• The only additional costs occur if patients chooses to utilize our contracted cash price lab arranged negotiated between us and local labs. The cost billed to the patient is equal to our cost for the labs and transaction fee
  
• There Are No Co-pays Ever
  
• Upon request we can supply a "Superbill" that patients can submit to their insurance company for potential reimbursement
